In consideration of being allowed to participate in any and all activities of the Sligo Creek Cooperative (hereinafter referred to as “SCC”), I:

Agree that I should inspect the facilities and equipment to be used, and that if I believe anything to be unsafe, I should immediately advise a board member of such condition(s) and refuse to participate. If a board member is unavailable, I should inform the adult performing Door Duty.
Acknowledge and fully understand that I may be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from our own actions, inactions, or negligence but the action, inaction, and negligence of others, or the condition of the premises or of any equipment used. Further, I acknowledge that there may be other risks not known to us or not reasonably foreseeable at this time.
Assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death.
Release, waive, discharge and covenant not to sue SCC, its administrators, directors, agents, and other employees of the organization, other members/participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of the premises used to conduct SCC activities, all of which are hereinafter referred to as “releases,” from any and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses or damages on account of injury, including death and damage to property, caused or alleged to be caused in whole or in part by the negligence of the releases or otherwise.
I, the undersigned, hereby represent that I have consented to participation in SCC and have agreed to the terms of the Waiver and Release of Liability set forth above. I further agree to save, hold harmless and indemnify each and all of the parties referred to above from all liability, loss, cost, claim or damage whatsoever which may be imposed upon said parties because of any defect in or lack of such capacity to so act and release said parties.

You agree your electronic signature is the legal equivalent of your manual/handwritten signature on this Agreement. You will not, at any time in the future, repudiate the meaning of your electronic signature or claim that your electronic signature is not legally binding.
